ownership;
a certificate of registration of a licence;
a utility model certificate;
a certificate of registration of an industrial
design; or

(2) A request for a copy of a lost or destroyed
certificate shall be in Form IP 42.
(3) A request for a copy of a lost or destroyed
certificate shall be accompanied by a statutory
declaration or affidavit supporting the assertion
that the certificate has been lost or destroyed.
Request for a
search.
70. A request to have a search done for an industrial
property right shall be in Form IP 43.
Request to amend a
name, etc. in a
register.
71. A request to amend a name, address or other contact
information in a register shall be in Form IP 44.
Caution or notice
in a register.
72. A request to have a caution or notice included in or
removed from a register shall be in Form IP 45.

73. (1) A notice or other document that is to be given or
sent to, or served on, a person may be given, sent or
served —
(a)
by mailing or delivering the notice or
document to the address of service
provided by the person;
(b)
by giving the notice or document
personally to the person or by mailing or
delivering the notice or document to the
person’s residence or place of business or
employment; or
